We are currently seeking an experienced and enthusiastic Primary Care Nurse who has at least Chronic Disease Management experience in either Diabetes or Asthma. This is a fantastic opportunity to join our dynamic team of nurses, healthcare assistants, and phlebotomists.

Salary and Benefits: The estimated salary for this position is £35,000 - £45,000 depending on experience. We also offer a range of benefits including further training opportunities and a supportive working environment.

Job Description: As a Primary Care Nurse, you will be responsible for delivering high-quality care to our patients. Your duties will include managing chronic diseases such as Diabetes and Asthma, and working closely with the healthcare team to provide enhanced services.

Requirements: To be successful in this role, you will need to have proven experience in a primary care setting, with a strong focus on Chronic Disease Management. You should be keen to keep yourself updated on new challenges facing primary care and be prepared to undertake further training as required.
