About the Role

We are seeking an experienced Occupational Therapy Care Manager to join our Community Independence Service (CIS) team. As a key member of our Reablement Team, you will play a vital role in delivering clinically appropriate care in the community, promoting health and independence among our service users.

Key Responsibilities
  • Deliver skilled Occupational Therapy interventions to individuals in their own homes, assessing eligibility for long-term care and support.
  • Manage a caseload, working closely with Reablement care providers, and providing goal plans and feedback.
  • Implement moving and handling plans, assist with hospital discharges, and undertake a duty OT role on a rotational basis.
  • Demonstrate and instruct service users and their families on equipment use and maintenance, identifying safeguarding concerns and working in partnership with voluntary and statutory agencies.
About You

We are looking for a highly skilled and experienced Occupational Therapist with a degree or equivalent, HCPC registration, and experience working in a multi-disciplinary setting with adults with disabilities. You will possess excellent communication, interpersonal, and problem-solving skills, with proficiency in IT programs and software, including Mosaic, System1, and the MS Office suite.
