Occupational Therapy Care Manager

We are seeking an experienced Occupational Therapy Care Manager to join our Community Independence Service (CIS) team. As a key member of our Reablement Team, you will play a vital role in delivering clinically appropriate care in the community, promoting health and independence among our service users.
Key Responsibilities- Deliver skilled Occupational Therapy interventions to individuals in their own homes, assessing eligibility for long-term care and support.
- Manage a caseload, working closely with Reablement care providers, and providing goal plans and feedback.
- Implement moving and handling plans, assist with hospital discharges, and undertake a duty OT role on a rotational basis.
- Demonstrate and instruct service users and their families on equipment use and maintenance, identifying safeguarding concerns and working in partnership with voluntary and statutory agencies.
We are looking for a highly skilled and experienced Occupational Therapist with a degree or equivalent, HCPC registration, and experience working in a multi-disciplinary setting with adults with disabilities. You will possess excellent communication, interpersonal, and problem-solving skills, with proficiency in IT programs and software, including Mosaic, System1, and the MS Office suite.
