Care Specialist

University Hospitals Dorset NHS Foundation Trust is seeking a skilled Palliative Care Specialist to join our team. As a Palliative Care Specialist, you will provide expert nursing care and symptom control advice to patients with advanced, progressing life-limiting illnesses, and their families.
Main Responsibilities:- Provide nursing expertise and palliative care advice in symptom control and psychological support for patients with advanced, progressing life-limiting illness with complex needs, and their families under regular supervision.
- Act as an expert resource for patients, families/carers and health and social care professionals caring for them.
- Support Advance Care Planning and other sensitive conversations utilising good communication skills.
- Promote collaborative working and personalised care and support 'what matters most' to patients and their families.
- Manage a clinical caseload of patients under supervision and proactively support and co-ordinate the individualised care and support needs for patients and their families acting as patient advocate, through collaboration and participation with the rest of the multi-disciplinary team within primary and secondary care.
- Palliative Care experience
- Experience of multi-professional working
- Ability to manage own workload effectively and independently
- Post registration qualification in palliative care or relevant subject or working towards
