About the RoleWe are seeking an experienced Community Support Specialist to join our team of dedicated professionals. As a key member of our community-based services, you will provide housing-related support to adults, promoting positive lifestyles and assisting individuals in developing essential skills for independent living.
Working closely with our partners across the county, you will employ a multi-agency approach to achieve optimal outcomes for our clients. Your role will involve acting as a keyworker, providing guidance and support to individuals, and working on our internal IT system to ensure accurate recording and timely updates.
Key Responsibilities- Deliver high-quality support services to adults in our community-based programs.
- Collaborate with internal and external stakeholders to develop and implement effective support plans.
- Maintain accurate records and reports using our internal IT system.
To be successful in this role, you must possess excellent communication and interpersonal skills, a can-do attitude, and the ability to work independently as well as part of a team. A full UK driving licence and access to a vehicle are essential for this position.
