About the Role

We are seeking a dedicated and compassionate Social Worker to join our Chesterfield Long Term Adult Care team at Derbyshire County Council.

Key Responsibilities
  • Conduct thorough person-centred assessments to understand individual needs and preferences.
  • Work closely with adults and their carers to gather information and promote positive wellbeing.
  • Undertake safeguarding enquiries, demonstrating strong communication and organisational skills.
  • Collaborate with other organisations and professionals to foster effective multi-disciplinary practice.
  • Support individuals in maximising opportunities for choice and control over their lives.

Support and Work Environment

Opportunity for home and office working.

Support provided through Microsoft Teams chat facility, daily catch-ups with team members and Senior Practitioner/Service Manager, monthly one-to-one supervision, and team meetings.

'Champion' role available for developing practice interests and sharing knowledge within the team.
