Advanced Nurse Practitioner Job Opportunity

Service Care Solutions Ltd is seeking an experienced Advanced Nurse Practitioner to join our team in the North West region. As a key member of our 111 Telephone Triage team, you will be responsible for delivering high-quality nursing services to patients, working closely with a multidisciplinary team to provide care within your scope of practice.

Key Responsibilities:
  • Deliver evidence-based nursing services to patients, including health promotion, chronic disease management, and health prevention.
  • Work as part of the service multi-disciplinary team to develop, implement, and evaluate individual treatment plans for acute and non-acute conditions.
  • Support the clinical team in the development of individual treatment plans for chronic disease patients and identify, manage, and support patients at risk of developing long-term conditions.
  • Provide routine nursing care to patients as required, in accordance with clinical-based evidence, NICE, and the NSF.
  • Prescribe medication in accordance with PGD and within your own scope of practice, and review medication for effectiveness, need, and national guidance.
