Job Title: Advanced Nurse Practitioner

Service Care Solutions Ltd is seeking an experienced Advanced Nurse Practitioner to join our team in the North West region. As a key member of our 111 Triage service, you will be responsible for delivering high-quality nursing services to patients, working autonomously and as part of a multi-disciplinary team.

Key Responsibilities:
  • Deliver evidence-based nursing services to patients, including health promotion, chronic disease management, and health prevention.
  • Work collaboratively with the clinical team to develop and implement individual treatment plans for acute and non-acute conditions.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
  • Provide routine nursing care to patients, adhering to clinical guidelines, NICE, and NSF standards.
  • Prescribe medication in accordance with PGD and within your scope of practice, reviewing medication for effectiveness, need, and national guidance.
