Job Title: Advanced Nurse Practitioner

Service Care Solutions Ltd is seeking an experienced Advanced Nurse Practitioner to join our team in the North West region. As a key member of our 111 Telephone Triage team, you will be responsible for delivering evidence-based nursing services and working collaboratively with a multi-disciplinary team to provide high-quality patient care.

Key Responsibilities:
  • Deliver evidence-based nursing services to patients, working within your scope of practice.
  • Collaborate with the multi-disciplinary team to develop and implement individual treatment plans for acute and non-acute conditions.
  • Support the clinical team in the development of treatment plans for chronic disease patients and identify, manage, and support patients at risk of developing long-term conditions.
  • Provide routine nursing care to patients, adhering to clinical guidelines, NICE, and NSF standards.
  • Prescribe medication in accordance with PGD and within your scope of practice, reviewing medication for effectiveness, need, and national guidance.
Requirements:
  • Experience as an Advanced Nurse Practitioner in a similar role.
  • Strong clinical skills and knowledge of nursing practices.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
