Job Title: Advanced Nurse Practitioner

Service Care Solutions Ltd is seeking an experienced Advanced Nurse Practitioner to join our team in the North West region. As a key member of our Home Visiting Service, you will be responsible for delivering evidence-based nursing services to patients in their homes.

Key Responsibilities:
  • Deliver high-quality nursing care to patients in their homes, working autonomously as part of a multi-disciplinary team.
  • Prescribe medication in accordance with PGD and within your scope of practice.
  • Review medication for effectiveness, need, and national guidance.
  • Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions.
  • Support the clinical team in developing individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long-term conditions.
Requirements:
  • Experience as an Advanced Nurse Practitioner or Clinical Practitioner.
  • Ability to work autonomously and as part of a team.
  • Strong communication and interpersonal skills.
  • Ability to prescribe medication in accordance with PGD and within your scope of practice.
