About the Role

We are seeking a skilled and compassionate Social Care Professional to join our Adult Services Mental Health Team in Lancashire. As a key member of our team, you will play a vital role in supporting adults with mental health difficulties to achieve their goals and live independently.

Key Responsibilities
  • Undertake and review assessments of need, risk, and capacity to ensure the best possible outcomes for service users.
  • Sourcing and commissioning care to meet the needs of service users, while working collaboratively with internal and external professionals, community services, carers, and service users.
  • Support discharge planning and work as part of the wider multi-disciplinary team to ensure seamless care.

Requirements

To be successful in this role, you will need to be registered with Social Work England and have a strong understanding of adult mental health services. You will also need to be able to work collaboratively as part of a team and have excellent communication and interpersonal skills.
