Job Overview

This is an exciting opportunity to lead and manage the Gwynedd Community Health team at Betsi Cadwaladr University Health Board. The post holder will be responsible for ensuring interfaces between the Local Authority and neighbouring services within the Division, including community mental health teams, in-patient services, and other statutory agencies and the voluntary sector.

Main Duties of the Job

The post holder will ensure that the team delivers evidence-based, safe, effective interventions and clinical care, following the nursing process. They will also be responsible for monitoring the interaction between staff and service users, ensuring that they are treated with dignity, kindness, and compassion at all times.

Accountability and Performance Management

The post holder will be accountable for the performance management of the team, relating to KPIs and outcome measures. They will also be responsible for reviewing and responding to Datix and safeguarding, workforce issues, and performance, which will be reported to the service manager.
