Job Overview

This is an exciting opportunity to lead and manage the Gwynedd Community Health team at Betsi Cadwaladr University Health Board. As a Health Team Manager, you will be responsible for ensuring interfaces between the Local Authority and neighbouring services within the Division, including community mental health teams, in-patient services, and other statutory agencies and the voluntary sector.

The post holder will develop the team to work flexibly, proactively, and responsibly to develop and manage clinical care for service users in Gwynedd, supporting the needs of the service. The team will deliver evidence-based, safe, effective interventions and clinical care, following the nursing process.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Main Duties of the Job

The post holder will ensure that the team follows the service principles and strategy for people with a learning disability while delivering safe, effective, and high-quality specialist services in line with agreed standards. The post holder will have excellent communication skills and be a visible and accessible manager to whom staff, patients, and their families can access for assistance, advice, and support.

The post holder will monitor the interaction between staff and service users, ensuring they are treated with dignity, kindness, and compassion at all times.

Accountability

The post holder will be accountable for the performance management in the team relating to KPIs and outcome measures, ensuring that fundamentals of care are in place and audits are completed where relevant. The post holder will communicate and receive highly complex and sensitive information and be able to analyse and interpret information and act appropriately where there may be barriers to acceptance.

The post holder will review and respond to Datix and safeguarding, workforce issues, and performance, which will be reported to the service manager. The post holder will have a good understanding of the CHC process and be part of the development of pathways and Maturity Matrix.
