We are seeking a highly skilled and compassionate Nursing Practitioner to join our team in a community setting, providing expert care and support to individuals in need.

- Deliver high-quality nursing care to individuals in their homes and communities, ensuring safe and effective delivery of services.
- Monitor patient conditions and health needs on a continuous basis, identifying and addressing any concerns through collaborative working.
- Develop and maintain effective relationships with patients, families, and carers, providing clear information and guidance.
- Contribute to the development of new care initiatives and programs, promoting excellence in community care.
We offer a competitive salary of £27,000 - £33,000 per annum, depending on experience, plus a range of benefits including ongoing training and development opportunities.
