Job Summary

We are seeking a highly skilled and compassionate Nursing Practitioner to join our team in a community setting. As a key member of our healthcare team, you will be responsible for providing high-quality care to individuals in their homes and communities.

  • Monitor patient conditions and health needs on a continuous basis, working collaboratively with healthcare teams to identify and address any concerns.
  • Communicate effectively with colleagues, sharing information and instructions to ensure seamless care coordination.
  • Recognize and report any situations or behaviors that may compromise patient safety or well-being.
Requirements
  • A qualified nursing professional with experience in community care settings.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders.
  • Ability to prioritize tasks effectively, manage time efficiently, and maintain accurate records.
  • Commitment to maintaining confidentiality and adhering to Trust policies and procedures.

  • A competitive salary of £25,500 - £31,400 per annum (dependent on experience).
