About the Role

We are seeking a highly skilled and motivated Medicines Management Specialist to join our Exeter Central and East Urgent Community Response team. As a key member of our multidisciplinary team, you will provide a safe and cost-effective medicines management pharmacy service to patients in their own homes.

Key responsibilities include:

  • Providing clinical assessment and review of medication-related needs for patients on the Urgent Community Response caseload
  • Offering expert advice to colleagues and stakeholders on medicines optimisation for patients
  • Promoting self-administration of medication through adherence interventions for individual patients
