Nurse Specialist

We are seeking a dedicated and compassionate Registered Nurse to join our Care of the Elderly team at the Countess of Chester Hospital NHS Foundation Trust.
As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care to our elderly patients, working collaboratively with other healthcare professionals to ensure the best possible outcomes.
With a strong focus on clinical excellence and patient-centered care, we are looking for a nurse who is passionate about delivering exceptional care and making a positive impact on the lives of our patients.
Key Responsibilities- Provide high-quality patient care to elderly patients, including assessment, planning, implementation, and evaluation of care.
- Work collaboratively with other healthcare professionals to ensure seamless care delivery and effective patient flow.
- Develop and maintain strong relationships with patients, families, and carers to ensure their needs are met and they feel supported.
- Participate in multidisciplinary team meetings to discuss patient care and develop care plans.
- Contribute to the development and implementation of care pathways and policies to improve patient outcomes.
