Job Summary

We are seeking an experienced Advanced Nurse Practitioner to join our team at Primary Care Management Solutions Ltd. As an autonomous practitioner, you will be responsible for delivering high-quality nursing services to our patients, working collaboratively with our multidisciplinary team to provide comprehensive care.

Main Responsibilities
  • Deliver practice nursing services, including health promotion, chronic disease management, and health prevention.
  • Work as part of the practice multidisciplinary team to provide care to the entitled patient population.
  • Support the management team in reviewing clinical policy and procedure.
  • Participate in audit as directed by the audit lead.

Requirements
  • Registered Nurse qualification.
  • Postgraduate diploma or degree in Advanced Practice Qualification.
  • Qualified Nurse Prescriber.
  • Qualified Triage Nurse.
  • Experience working in a primary care environment.
  • Experience in chronic disease management.
  • Experience in prescribing and undertaking medication reviews.
Clinical Knowledge
  • Wound Care / Removal of sutures & staples.
  • ECGs.
  • Venepuncture.
  • New patient medicals.
  • Chaperone procedure.
  • Requesting pathology tests and processing the results, advising patients accordingly.
  • Travel medicine.
  • Diabetes.
  • Hypertension.
  • Asthma.
  • Spirometry.
  • CHD.
  • Immunisations (routine, childhood and travel).
  • Women's health (Cervical cytology, contraception, etc.).
