Job Summary
We are seeking an experienced Advanced Nurse Practitioner to join our team at Primary Care Management Solutions Ltd. As an Advanced Nurse Practitioner, you will be responsible for delivering high-quality patient care in a primary care setting.
Main Responsibilities
The successful candidate will be responsible for:
- Providing autonomous practice nursing services, working as part of the practice multidisciplinary team
- Delivering care within their scope of practice to the entitled patient population
- Supporting the management team in reviewing clinical policy and procedure
- Working in clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics

Person Specification
The ideal candidate will have:
- Registered Nurse qualification
- Postgraduate diploma or degree in Advanced Practice Qualification
- Qualified Nurse Prescriber and Qualified Triage Nurse
- Experience working in a primary care environment, with a focus on chronic disease management and prescribing
- Strong clinical knowledge, including wound care, ECGs, venepuncture, and health promotion strategies
