About the Role

We are seeking two motivated mental health practitioners to join our team as Discharge Coordinators in Cheshire West and Wirral. The successful candidates will work closely with community care coordinators, FirstResponse Services, and inpatient teams to identify barriers to discharge for CWP inpatients.

The post holder will be expected to work as part of the Urgent Care Mental Health First response service, which includes Crisis Triage and response services, Liaison Psychiatry, Home Treatment teams, Street Triage and Liaison and Diversion Services.

Our ideal candidate will have a recognised qualification in mental health, such as RMN, AHP, Psychology, or Social care (AMHP), and experience working within mental health services. They will be passionate about supporting people to further their recovery and ensuring that hospital stays are only necessary when absolutely required.

We offer a competitive salary of £35,000 - £42,000 per annum, depending on experience, and a relocation package of up to £8,000 for those who need to relocate. In addition, we provide up to three weeks' induction training with our Prepare to Care programme for all new starters.

  • Essential: Recognised qualification in mental health (RMN, AHP, Psychology, Social care)
  • Desirable: Evidence of further professional development involving serious mental illness, willingness to undertake nurse prescribing certificate, non-medical prescribing certificate, sound knowledge of current professional staff development and continuous service improvement issues, particularly First Response and Acute Inpatient/Home Treatment Services
