Patient Care Navigator

About the Role:
We are seeking an enthusiastic individual to join our dynamic team as a Patient Care Navigator. This exciting opportunity has arisen at South Tees Hospitals NHS Foundation Trust, where you will be working alongside established teams of clinicians, specialist nurses, allied health professionals, and MDT and cancer care co-ordinators.
Main Responsibilities:
- Coordinating care from referral to treatment
- Supporting patients, families, and carers to ensure effective progress along care pathways
- Attending multi-disciplinary team meetings and transcribing outcomes using NHS IT recording systems
Requirements:
- NHS experience or equivalent
- Excellent communication skills and ability to interpret written instructions
- Experience with IT systems, particularly Microsoft Word, Excel, and Powerpoint
Salary: £26,000 - £35,000 per annum (dependent on experience)
