Key Responsibilities
  • Coordinate and manage the daily schedules of care workers to ensure timely and efficient service delivery.

  • Maintain accurate records and documentation of care provided, ensuring compliance with regulatory requirements.

  • Liaise with external agencies and professionals to ensure a cohesive approach to care, promoting collaboration and communication.
  • Conduct assessments of new clients and reassessments of existing clients to ensure their needs are met, making recommendations for changes to care plans as necessary.
Requirements
  • Previous experience in a care coordination role is essential, with a proven track record of delivering high-quality care and support.
  • NVQ Level 2/3 in Health and Social Care is preferable, demonstrating a strong understanding of care principles and practices.
  • Strong understanding of CQC regulations and compliance, ensuring adherence to regulatory requirements.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, staff, and external agencies.
  • Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a fast-paced environment.
  • Compassionate and empathetic nature, with a commitment to providing high-quality care and support.
  • Proficiency in using electronic care planning systems, with the ability to learn new systems and technologies as required.
  • Strong organisational and multitasking skills, with the ability to prioritize tasks and manage multiple responsibilities.
