Key Responsibilities- Conduct thorough assessments of individuals in need of social care services to determine the level of support required.
- Gather information from various sources to identify the needs of our service users and their families.
- Collaborate with social workers, healthcare providers, and the NHS to ensure that our service users receive the best possible care in accordance with the Care Act.
- Work closely with the Team Manager and Senior Social Worker to identify the needs of our service users and their families, create care plans, provide access to appropriate services and support, and monitor and review care plans to ensure they are meeting individuals' needs effectively.
The London Borough of Sutton is undergoing a significant transformation to address the rising demand for our adult social care services. About You- You will have a strong background in social care, with a minimum of NVQ level 3 or equivalent experience working with vulnerable people/families/carers.
- You will have prior experience in conducting assessments in a social care setting and be familiar with relevant legislation and policies in social care.
- You will be able to work collaboratively in a multidisciplinary team and have excellent communication skills to effectively interact with vulnerable people, their families, and healthcare professionals.
- You will have an understanding of issues related to safeguarding and protection of vulnerable adults and be committed to continuous professional development to stay updated on best practices in the field of social care.
