About the Role
We are seeking a highly motivated and caring Practice Nurse to join our team at Holmhurst Medical Centre. As a Practice Nurse, you will play a vital role in providing high-quality nursing care to our patients.
Key Responsibilities
- Administer adult and child immunisations and vaccinations in accordance with national and local programmes.
- Undertake procedures such as venepuncture, basic wound care, BP management, urinalysis, peak flow, spirometry, stitch/clip removal, and cervical smears.
- Work within all relevant practice policies and procedural guidelines, including infection control, chaperoning, and patient group directions.
- Initiate the process of diagnosis with patients suspected to have a chronic disease, such as diabetes, COPD, asthma, or CHD, and refer to other clinicians as appropriate.
- Manage programmes of care for patients with chronic diseases by planning, providing, and evaluating care under agreed local/national guidelines, protocols, and patient group directions.
- Contribute to a satisfactory call and recall system of patients and undertake annual review of patients.
- Work collaboratively as a team member to achieve the objectives of the NHS contract and aspirations of the team.
- Provide expert clinical advice on educational opportunities to facilitate the development and support of specialist knowledge and skills.
- Recognise the development needs of others and provide clinical supervision.
- Develop own knowledge and practice to meet objectives/changes in service.
- Make effective use of the clinical system by ensuring the consistent and accurate entering of data and use of SNOMED Codes.
About Us
Holmhurst Medical Centre is a purpose-built modern building with a pharmacy on site, located on the edge of the Surrey countryside. We are a member of a PCN and Dorking Healthcare Federation, with a list size of 10,200 patients. We are a training practice, CQC rated Good, and consistently achieve high QoF achievement. We have a multidisciplinary team committed to our local community, with Midwife and District Nurse support, IAPT, and onsite consultant clinics. We are supported by excellent nursing and administration teams and provide a range of services, including minor ops and offsite clinics in a school and nursing home.
