Job Overview

The Access Team is a dynamic and fast-paced service that provides a single point of entry and first-level gatekeeping into mental health services in Stockport. Our team screens and triages all referrals, undertaking telephone triages, urgent and routine face-to-face assessments, and onward referrals to the most appropriate service. Our goal is to get people the right care, at the right time, and by the right service.

Main Duties and Responsibilities
  • Conduct comprehensive assessments of individuals' health and social care needs, including risk assessments.
  • Coordinate assessment clinics and prioritize appointments.
  • Develop individualized packages of care, including signposting where necessary, within the stepped care model.
  • Liaise and negotiate with other professions and agencies to ensure the best possible service is provided.
  • Assist individuals in managing their emotional distress using psychological evidence-based interventions.
  • Provide education and self-help resources to patients specific to their needs.
  • Keep and store up-to-date written records and follow Pennine Care policies.
Working for Our Organisation

The Access Team operates from Monday to Friday, 9am to 5pm. We work closely with Healthy Minds, the Transition Team, and other internal and external providers. Staff are expected to participate in the overall development of the service, ensuring Clinical Governance principles underpin all developments.

Person SpecificationEducation and QualificationsEssential Criteria
  • RMN/RNMH/Social Worker equivalent.
  • Completion of Preparation for Mentorship or willingness to complete.
  • Evidence of ongoing CPD.
Desirable Criteria
  • Degree or working towards completion.
  • Non-medical prescribing qualification.
  • Formal teaching qualification.
  • Further training in brief psychosocial interventions.
ExperienceEssential Criteria
  • Significant post-registration experience.
  • Proven experience of working with clients with complex needs.
  • Experience of working with individuals in crisis and with symptoms of mental health illness.
  • Experience of working in multi-disciplinary teams.
  • Experience of providing supervision to junior staff.
Desirable Criteria
  • Primary care mental health work experience.
  • Experience in using outcome measures such as CORE, PHQ9, and GAD7.
  • Experience of working with a psychological model of care.
  • Experience of triaging referrals.
