Job Overview

We are seeking a skilled Mental Health Practitioner to join our team at Pennine Care NHS Foundation Trust. The successful candidate will be responsible for providing comprehensive mental health services to individuals in need, working closely with multidisciplinary teams to deliver high-quality care.

Key Responsibilities:
  1. Participate in the triage process of new referrals to the service.
  2. Carry out thorough assessments of service users' current mental health problems and individual needs.
  3. Develop individualized packages of care for service users with mental health problems.
  4. Liaise and negotiate with other professions to ensure the best possible service is provided to service users and carers.
  5. Provide education and appropriate self-help resources to patients specific to individual needs.


Working for Our Organisation

The team operates from Monday – Friday 9am – 5pm and works closely with Healthy Minds, The Transition Team, and other internal and external providers. Staff are expected to participate in the overall development of the service, ensuring that the principles of Clinical Governance underpin all developments. The successful candidate will be expected to deliver care in accordance with all Pennine Care NHS Foundation Trust policies and procedures, the Mental Health Act, and community care legislation.
