About the Role

We are seeking a skilled Community Health Nurse to join our team in the Anglesey and Gwynedd Area. As a Looked After Children Specialist, you will work closely with the LAC Nursing Team to provide high-quality health assessments and care plans for children in care.

Main Responsibilities

As a Child Health Specialist, you will be responsible for coordinating statutory health assessments and care plans, working in partnership with multi-disciplinary teams, including School Nursing, Health Visiting, and Community Paediatricians. You will be an advocate for Looked After Children and Care Leavers, ensuring the best possible health and social outcomes.

Requirements

To be successful in this role, you will need to be a registered nurse with experience in community nursing. Excellent record-keeping skills and the ability to work well with other agencies are essential. Welsh language skills are desirable, but not required.
