We are seeking a highly motivated and compassionate individual to join our team as an Adult Services Coordinator.

  • Assess the needs of older adults and individuals with physical disabilities
  • Develop and implement care plans that promote independence, well-being, and quality of life
  • Work collaboratively with service users, their families, and other professionals
  • Maintain accurate and up-to-date case records and reports
  • Ensure compliance with relevant legislation, policies, and procedures

Salary and Benefits:
