Our Opportunity

This is an Enhanced Care Practitioner role where you will provide holistic care to enhance the wellbeing and outcomes of individuals with complex needs.

You will engage with service users, assess their needs, and develop tailored plans to address their challenges.

Your work will involve outreach support, home visits, and liaison with specialist teams to provide comprehensive care.

  • Assess and engage with service users requiring flexible support due to multiple needs.
  • Manage a caseload, reviewing ongoing care and treatment with specialist teams and stakeholders.
  • Conduct home visits, support clients with appointments, and meet individuals in the community.
  • Collaborate with the multi-disciplinary team, providing advice and guidance through reviews, meetings, and supervision.
Requirements
  • You must have a full clean UK driving licence and access to your own vehicle.
