Community Health Specialist

We are seeking a skilled and compassionate Community Nurse to join our team at Cambridgeshire & Peterborough NHS Foundation Trust. As a Community Nurse, you will play a vital role in delivering high-quality nursing care to patients in their own homes or residential settings.
Main Responsibilities- Assess and develop care plans for patients with complex needs
- Provide wound care, palliative care, and long-term condition management
- Work collaboratively with multidisciplinary teams to achieve patient-centered outcomes
- Develop and maintain relationships with patients, families, and carers
- Contribute to the development of community-based services and initiatives
