Care Support Specialist

We are seeking a dedicated and compassionate Care Support Specialist to join our team at Community Integrated Care. As a Care Support Specialist, you will play a vital role in providing high-quality care and support to individuals in their own homes.
Your primary responsibility will be to deliver personalized care and support plans, ensuring that each individual receives the best possible care and support to meet their unique needs and goals.
You will work closely with our team of experienced care professionals to develop and implement care plans, provide emotional support, and promote independence and well-being.

Key Responsibilities:
- Deliver personalized care and support plans to meet individual needs and goals
- Work closely with care teams to develop and implement care plans

- Support individuals with daily living skills, including personal care and medication management
- Communicate effectively with individuals, families, and care teams to ensure seamless care delivery
