We are seeking an experienced leader to join our team as Population Health Management Director. This pivotal role will play a crucial part in driving the transformation of health, care, and well-being outcomes for the citizens of Derbyshire.

Job Overview

NHS Derby and Derbyshire Integrated Care Board (ICB) is a statutory body responsible for planning and delivering joined-up services across the Derby City and Derbyshire population. Our ICB has four overarching aims: to improve outcomes in population health and healthcare, and to help the NHS support broader social and economic development.

This exciting opportunity will see you lead our Population Health Management function, working closely with our Deputy Chief Medical Officer and the medical directorate to deliver high-quality, safe, and effective care across care pathways within Joined Up Care Derbyshire. You will drive an evidence-informed approach to care, transforming health, care, and well-being outcomes for the citizens of Derbyshire.

Main Duties
  • Lead the Population Health Management function, developing and implementing strategies to improve population health and reduce health inequalities.
  • Work closely with our Deputy Chief Medical Officer and the medical directorate to deliver high-quality, safe, and effective care across care pathways.
  • Develop and maintain relationships with key stakeholders, including local authorities, clinical commissioning groups, and other healthcare providers.
  • Monitor and evaluate the impact of Population Health Management initiatives, making recommendations for improvement.
