Key Responsibilities- Develop and maintain relationships with service users, their families, and other stakeholders to ensure a person-centred approach to care.
- Assist in the development and implementation of individualised care plans, including activity analysis and graded interventions.
- Coordinate and deliver a range of activities and interventions to promote physical and mental wellbeing, including group and one-to-one sessions.
- Contribute to the assessment and risk management process, ensuring a safe and supportive environment for service users.
- Work collaboratively with the multidisciplinary team to achieve positive outcomes for service users.
- Care certificate or willingness to undertake a similar qualification.
- Ability to motivate and engage service users, with a strong focus on building therapeutic relationships.
- Excellent communication and interpersonal skills, with the ability to work effectively with a diverse range of stakeholders.
- Ability to deliver groups and one-to-one enrichment activities, with a focus on promoting independence and choice.
- Full UK driving licence is desirable.
- Annual salary of £25,272.
