Housing Specialist Case Manager

Cih International Housing group is looking for an enthusiastic Housing Specialist Case Manager to provide intensive one-to-one support to individuals who are homeless or at risk of becoming homeless in Bournemouth, Poole, and Christchurch.
The ideal candidate will have a strong understanding of best practice in supporting individuals with complex needs, including substance misuse and mental health issues. As a key member of our team, you will work closely with clients to identify their strengths and talents, and develop personalized support plans to help them achieve their goals.
About the JobThis is a rewarding full-time position offering a competitive salary of £24,000 per year, depending on experience. You will work a 37-hour week, with a rolling rota that includes early, mid, and late shifts. This role requires staff to work some weekends and involves lone working in the community.
Main Responsibilities- Provide intensive one-to-one support to clients, focusing on addressing their immediate needs and long-term goals.
- Develop and implement individualized support plans to address client needs.
- Liaise with outreach workers and other agencies to build trust and rapport with clients.
- Encourage clients to engage in services that improve their physical and mental health.
- Help clients access benefits they are entitled to claim.
- Work collaboratively with external partners to maximize opportunities for clients.
