About the Role:

We are seeking a skilled and compassionate Care Pathways Coordinator to join our team at Look Ahead Care and Support. This is an exciting opportunity for someone who is passionate about making a difference in the lives of vulnerable adults with Learning Disabilities.

The successful candidate will be responsible for developing and maintaining relationships with private sector landlords and estate agents, promoting Look Ahead as a partner organisation. They will also work closely with local authorities to create innovative schemes that support long-stay residents in accommodation. Additionally, they will attend placement review meetings, maintain accurate records of service utilisation, and develop information resources to support operational staff in the resettlement of customers. This includes details on Local Housing Allowance Rates, Partner Landlord and Estate Agency information, and Rent Deposit Schemes. The coordinator will serve as the main point of contact for information and resources on the private rented sector. This role requires excellent networking and partnership skills, as well as the ability to manage multiple tasks and priorities effectively.
