Adult Mental Health Care Coordinator

Cheshire West and Chester Council is seeking a dedicated professional to join the Adult Mental Health Prevention & Wellbeing Team.
This dynamic team delivers rehabilitation, treatment programs, care planning, assessment, and long-term focus on recovery and overall wellbeing for individuals with complex needs.
The ideal candidate will have a passion for delivering creative and innovative solutions in a community setting. They will be able to assist with ongoing activity programs and share experiences in a group setting, promoting varied stimulus, structure, and inclusion.
Key Responsibilities:
- Delivering frontline care, including mental health support
- Identifying and coordinating services to meet individual needs
- Working knowledge of care provision processes, including Health & Safety and Risk Assessments
- Applying Recovery principles to enhance service delivery
A competitive salary of £30,000 - £40,000 per annum is offered, along with a generous holiday allowance, career average pension scheme, and rewards for going above and beyond.
