Learning Disability Care Coordinator

We are currently seeking a Learning Disability Care Coordinator to join our team at Cheshire and Wirral Partnership NHS Foundation Trust. As a Learning Disability Care Coordinator, you will play a crucial role in supporting individuals with learning disabilities, promoting their independence, and improving their overall well-being.
The ideal candidate will possess a strong understanding of learning disabilities, autism spectrum condition, and mental health. They will also have excellent communication skills, IT proficiency, and experience of working in a variety of settings, including homes, day services, and hospitals.
This role offers a competitive salary of £24,907 - £30,615 per annum (inclusive of annual leave allowance). In addition to the salary, the successful candidate will have access to ongoing training and development opportunities, a Guaranteed Interview Scheme for armed forces community applicants, and our Prepare to Care induction program.
Key responsibilities of the role include supporting clients in crisis, conducting assessments, and developing treatment plans. The successful candidate will also have the opportunity to deliver training to other professionals, service users, and carers.
