About The Role

We are seeking a highly motivated and empathetic Support Worker to join our team at The Disabilities Trust. As a Support Worker, you will play a vital role in supporting individuals with acquired brain injury, physical disabilities, and/or sensory impairment to live independently.

Key Responsibilities

  • Provide person-centered support to individuals, promoting their independence and well-being
  • Assist with daily living tasks, such as personal care and household chores
  • Encourage and support individuals to participate in activities and therapies
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans
  • Monitor and report any changes in an individual's condition or behavior

Requirements

  • Empathy and understanding of the needs of individuals with acquired brain injury and/or physical disabilities
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Good time management and organizational skills
  • Willingness to learn and develop skills in areas such as personal care and behavior support
