About The Role
We are seeking a highly motivated and empathetic Support Worker to join our team at The Disabilities Trust. As a Support Worker, you will play a vital role in supporting individuals with acquired brain injury, physical disabilities, and/or sensory impairment to live independently.
Key Responsibilities
- Provide person-centered support to individuals, promoting their independence and well-being
- Assist with daily living tasks, such as personal care and household chores
- Encourage and support individuals to participate in activities and therapies
- Work collaboratively with the multidisciplinary team to develop and implement individualized care plans
- Monitor and report any changes in an individual's condition or behavior
Requirements
- Empathy and understanding of the needs of individuals with acquired brain injury and/or physical disabilities
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Good time management and organizational skills
- Willingness to learn and develop skills in areas such as personal care and behavior support
