Job Role: Community Health Practitioner

We have an exciting opportunity for a skilled and compassionate Community Health Practitioner to join our team in the NHS. As an integral part of our community health services, you will play a vital role in providing high-quality care to vulnerable adults in their homes and communities.

A proven track record of working in a care setting, preferably in a physical health environment, is essential. We are looking for someone who can demonstrate flexibility, empathy, and a willingness to undertake training and development opportunities.

Key Responsibilities:
* Provide comprehensive support to patients and their families, ensuring seamless care delivery.
* Collaborate with healthcare professionals to develop and implement personalized care plans.
* Participate in risk assessments, identifying potential hazards and implementing mitigating strategies.
* Maintain accurate records, documenting patient progress and outcomes.

We offer a competitive salary of £33,000 - £36,000 per annum, depending on experience. This is a full-time position, but we also welcome applications from those interested in part-time or flexible working arrangements.
