Senior Primary Care Clinical Pharmacist

We are seeking an experienced Clinical Pharmacist to join our team at Alliance for Better Care CIC. As a Senior Primary Care Clinical Pharmacist, you will play a pivotal role in improving the quality of care and operational efficiencies in our general practice.
Key Responsibilities- Provide clinical leadership on medication optimization and quality improvement, managing some aspects of the quality and outcomes framework and enhanced services.
- Contribute to the delivery of the local medication optimization strategy, ensuring that the practice integrates with community and hospital pharmacy to improve patient outcomes and manage workload.
- Support general practice staff with prescription and medication queries, helping to optimize the repeat prescribing system and deal with acute prescription requests.
- Provide expertise in clinical medicines advice, addressing both public and social care needs of patients registered with the practice.
- Work with case managers, multidisciplinary review teams, hospital colleagues, and virtual ward teams to manage medication-related risk for readmission and patient harm.
- Produce and implement a practice repeat prescribing policy, managing the repeat prescribing reauthorization process, and ensuring patients have appropriate monitoring tests in place when required.
- Identify national and local policy and guidance that affects patient safety through the use of medications, including MHRA alerts, product withdrawals, and emerging evidence from clinical trials.
- Take a leading role in the contractual requirements to support frail patients with their medication, including medication review and patient identification.
- Work with the CCG Medicines Management Team to implement local and national guidelines, including NICE, and liaise with hospital colleagues where prescribing needs to be returned to specialists.
- Provide education and training to primary healthcare teams on therapeutics and medication optimization, as well as training to visiting medical, nursing, pharmacy, and other healthcare students.
- Work with the general practice team to ensure the practice is compliant with CQC standards where medications are involved, undertaking risk assessment and ensuring compliance with medication legislation.
- Support public health campaigns, providing specialist knowledge on all public health programs available to the general public.
The role may evolve to include managing own caseload of vulnerable housebound patients at risk of hospital admission and harm from poor use of medications, implementing own prescribing changes, and ordering monitoring tests.
Working with colleagues, identify and provide leadership on areas of prescribing and medication optimization, conducting clinical audits and medicine optimization projects, and presenting results and providing leadership on suggested changes.
Contribute to national and local research initiatives, ensuring that patients are referred to the appropriate healthcare professional for the right care within an appropriate period of time.
