Senior Primary Care Clinical Pharmacist

We are seeking an experienced Clinical Pharmacist to join our team at Alliance for Better Care CIC. As a Senior Primary Care Clinical Pharmacist, you will play a pivotal role in improving the quality of care and operational efficiencies in our general practice.
Key Responsibilities- Provide clinical leadership on medication optimization and quality improvement, managing some aspects of the quality and outcomes framework and enhanced services.
- Contribute to the delivery of the local medication optimization strategy, ensuring that the practice integrates with community and hospital pharmacy to improve patient outcomes and manage workload.
- Support general practice staff with prescription and medication queries, helping to optimize the repeat prescribing system and deal with acute prescription requests.
- Provide expertise in clinical medicines advice, addressing both public and social care needs of patients registered with the practice.
- Work with case managers, multidisciplinary review teams, hospital colleagues, and virtual ward teams to manage medication-related risk for readmission and patient harm.
- Produce and implement a practice repeat prescribing policy, managing the repeat prescribing reauthorization process, and ensuring patients have appropriate monitoring tests in place when required.
- Identify national and local policy and guidance that affects patient safety through the use of medications, including MHRA alerts, product withdrawals, and emerging evidence from clinical trials.
- Take a leading role in the contractual requirements to support frail patients with their medication, including medication review and patient identification.
- Work with the CCG Medicines Management Team to implement local and national guidelines, including NICE, and liaise with hospital colleagues where prescribing needs to be returned to specialists.
- Provide education and training to primary healthcare teams on therapeutics and medication optimization, as well as training to visiting medical, nursing, pharmacy, and other healthcare students.
- Work with the general practice team to ensure the practice is compliant with CQC standards where medications are involved, undertaking risk assessment and ensuring compliance with medication legislation.
Alliance for Better Care CIC is a GP Federation that unites 47 NHS GP practices across 12 Primary Care Networks in Sussex and Surrey. We support our Primary Care colleagues as well as their patients, to transform how healthcare is managed within the community.
Requirements- Mandatory registration with the General Pharmaceutical Council.
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
- Completed CPPE course.
- Minimum of 4 years post-qualifying experience.
- Demonstrates experience managing other pharmacists and pharmacist technicians.
- Experience of partnership/collaborative working and of building relationships across a variety of organizations.
- Meets DBS reference standards and has a clear criminal record.
- Safeguarding and other mandatory training such as BLS.
- Access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own homes.
