Senior Primary Care Clinical Pharmacist

Join our team as a Senior Primary Care Clinical Pharmacist and contribute to improving the quality of care and operational efficiencies in general practice. As a key member of our multidisciplinary team, you will work closely with healthcare professionals to provide expert clinical leadership on medicines optimisation and quality improvement.
About the RoleThis is an exciting opportunity for an experienced pharmacist to support a wide range of patients of all ages. You will work as part of a multi-disciplinary team in a patient-facing role, taking responsibility for areas of chronic disease management within the practice and undertaking clinical medication reviews to proactively manage patients with complex polypharmacy.
Key Responsibilities- Provide clinical leadership on medicines optimisation and quality improvement, managing some aspects of the quality and outcomes framework and enhanced services.
- Contribute to the delivery of the local medicines optimisation strategy, ensuring that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, and manage workload.
- Undertake clinical medication reviews with patients with multimorbidity and polypharmacy, implementing own prescribing changes and ordering relevant monitoring tests.
- Provide patient-facing clinics for those with questions, queries, and concerns about their medicines in the practice.
- Act as a source of medicines information for all of the practice team and patients, suggesting and recommending solutions.
- Reconcile medicines following discharge from hospitals, intermediate care, and into care homes, identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.
- Manage caseloads of patients with common/minor/self-limiting ailments, working within scope of practice and limits of competence, and signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.
- Ensure that patients are referred to the appropriate healthcare professional for the right care, ensuring that the addition of the pharmacist to the clinical team results in a reduction in workload for other clinicians.
- Work with case managers, multidisciplinary (health and social care) review teams, hospital colleagues, and virtual ward teams to manage medicines-related risk for readmission and patient harm.
- Produce and implement a practice repeat prescribing policy in line with the recommendations from the Medicines Management Team (MMT), managing the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review.
- Identify national and local policy and guidance that affects patient safety through the use of medicines, including MHRA alerts, product withdrawals, and emerging evidence from clinical trials, managing the process of implementing changes to medicines and guidance for practitioners.
- Take a leading role in the contractual requirements to support frail patients with their medication, including medication review and patient identification (patients with problematic polypharmacy).
- Work with CCG MMT to implement local and national guidelines, including NICE, liaising with hospital colleagues where prescribing needs to be returned to specialists.
- Provide education and training to primary healthcare team on therapeutics and medicines optimisation, providing training to visiting medical, nursing, pharmacy, and other healthcare students where appropriate.
- Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved, undertaking risk assessment and ensuring compliance with medicines legislation.
- Support public health campaigns, providing specialist knowledge on all public health programmes available to the general public.
