Job Summary

This is an exciting opportunity for an experienced and enthusiastic registered healthcare professional to join our team as a Clinical Support Manager for Continuing Healthcare (CHC). As a key member of our All-Age Continuing Care (AACC) service, you will provide professional leadership, advice, and expertise to ensure the delivery of high-quality clinical assessments, reviews, and management of AACC cases.

Main Responsibilities
  • Provide expert advice and guidance to support health and social care staff involved in patient assessment, care provision, and care and support planning.
  • Assess, care, and support planning, ongoing case management, and review for all patients eligible for NHS continuing care funding.
  • Support panels, appeals, and local resolution processes as a clinical representative.
  • Undertake or coordinate appropriate assessments, approvals, and notifications of deprivations of liberty, maintaining comprehensive records.
  • Ensure the agreed care and support package continues to meet the individual's assessed health and associated care/support needs and their agreed outcomes.
Requirements

We are looking for a candidate with substantial experience of working at a senior clinical level in AACC services, with a comprehensive understanding of the relevant NHS frameworks for AACC. The ideal candidate will have excellent communication and interpersonal skills, with the ability to provide and receive highly complex, sensitive, and contentious information.
