About the Role

We are seeking an enthusiastic and experienced healthcare support worker to join our dynamic and supportive team in Worcestershire. As a key member of our community learning disability team, you will work with adults who have a learning disability, providing support to ensure their health needs are met.

Key Responsibilities
  • Assess, monitor, and evaluate service users' health and support needs through discussions with carers/service users, direct observation, and liaison with other health providers.
  • Plan service users' care through discussions with the allocated community nurse.
  • Implement health-related interventions, train service users and their carers in their implementation, and contribute to monitoring and evaluating their effectiveness.
  • Support service users and their carers in implementing health interventions and in monitoring their service users' health.
  • Facilitate service users' access to medical and health appointments and support each service user through these appointments, ensuring that information is given to them in a way that they understand.
Requirements
  • Good standard of general education.
  • Level 3 qualification (e.g., A-level, NVQ3) in Health Care.
  • MAPA Trained or willing to undertake this training and update as required.
  • Ability to work flexibly and with minimal supervision.
  • Able to meet the travel requirements of the post.
