Nurse Care Coordinator

We are seeking a qualified Registered Nurse to join our team at Heeley Bank Care Home in Sheffield, S2. Our care home provides a supportive environment for older adults and individuals living with dementia.
The successful candidate will be responsible for delivering high-quality care to our residents, working closely with the care team to ensure person-centred care plans are implemented effectively. Job Summary:
- Formulate person-centred care plans and review/update them as necessary
- Administer prescribed medicines and document accordingly
- Monitor resident well-being and report any concerns to relevant healthcare professionals
- Supervise the care team when acting as nurse in charge
- Liaise with healthcare professionals to ensure seamless care delivery
Key Responsibilities:
As a Registered Nurse at Monarch Healthcare, you will play a vital role in ensuring the highest standards of care are met. Your duties will include:
- Reviewing and updating care plans to reflect client need
- Ensuring care plans are completed and maintained in consultation with clients, relatives, and other healthcare professionals
- Administering prescribed medicines and documenting the same
- Monitoring resident well-being and reporting any concerns to relevant healthcare professionals
- Supervising the care team when acting as nurse in charge
- Liaising with healthcare professionals to ensure seamless care delivery
