About the Role

We are seeking a highly skilled and experienced Clinical Team Leader to join our Acute Clinical Team (ACT) at Swansea Bay University Health Board. As a Clinical Team Leader, you will be responsible for leading a team of healthcare professionals in providing high-quality care to patients in their homes and other residential settings.

Key Responsibilities

  • Lead a team of healthcare professionals in providing care to patients in their homes and other residential settings.
  • Develop and implement care plans in collaboration with patients, families, and healthcare teams.
  • Provide clinical leadership and guidance to healthcare professionals in the team.
  • Monitor and evaluate the quality of care provided by the team.
  • Collaborate with other healthcare teams and services to ensure seamless care delivery.
  • Participate in the development and implementation of policies and procedures to improve patient care.
  • Contribute to the education and training of healthcare professionals in the team.

Requirements

  • NMC Registered.
  • Clear understanding of the consent process.
  • Ability to communicate effectively.
  • Can demonstrate SBUHB values.
  • Ability to work flexibly.
  • Clear understanding of contract of care.
  • Ability to travel within the geographical area.

Desirable

  • Evidence of post-study registration study and training.
  • Clear understanding of clinical governance framework.
  • Implement evidence-based practice.
  • Knowledge of incident reporting policy.
