Care Coordinator

We are seeking an experienced Care Coordinator to join our dynamic team at Active Care Group Recruitment. As a Care Coordinator, you will be responsible for delivering high-quality care services to our clients in their own homes.
Key Responsibilities:- Develop and implement care plans that meet the unique needs of each client
- Work closely with our clinical and business leadership teams to ensure seamless care delivery
- Build strong relationships with clients, families, and external stakeholders
- Ensure accurate and timely record-keeping and reporting
- Support the recruitment and training of support workers
- NVQ 4 or above in Health and Social Care
- Full clean driving license with access to own car
- Excellent communication and interpersonal skills
- Experience in care at Team Leader level or above
- Previous management experience would be an advantage
- Salary up to £30,000 per annum pro rata
- 25 days annual leave, plus birthday off
