Job Summary
The Care Coordinator will play a vital role in supporting patients with enhanced needs, ensuring they have access to the services and support required to improve their health and wellbeing. This includes proactively identifying and liaising with patients to support their personalised care requirements, making sure that appropriate support is made available to them and their carers.
Main Responsibilities
- Liaise with practice teams to ensure proactive call and recall systems for patients with Learning Disabilities, booking and completing annual health checks, and providing information to help them get the most from their review.
- Proactively engage with patients, carers, and practices to ensure maximum uptake of reviews, and follow up any patients who fail to attend appointments.
- Act as a link between Support Workers, the GP, and other healthcare colleagues, supporting onward referrals, care planning, and administrative planning needs as appropriate.
- Administer weight management services, identifying suitable patients, making contact, and booking appointments as required.
- Ensure safeguarding issues or patient-related concerns are raised appropriately to a suitable individual.
Requirements
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals. They will also have a strong understanding of healthcare systems and processes, with the ability to navigate complex care pathways.
