Health Visitor Assistant

The successful candidate will be responsible for co-ordinating and managing their own workload, delivering evidence-based practice to families, and implementing the Healthy Child Wales Contacts.
Apart from this, you will work within the community setting, assessing, planning, implementing, and evaluating care offered to families under the supervision of the Delegated Health Visitor.
