**About the Role**

We are seeking an enthusiastic and innovative Health Visitor Specialist to join our universal health visiting service in Connah's Quay, Flintshire area of North Wales.

The successful candidate will have professional responsibility for a defined population/caseload and lead the delivery & organisation of the public health program. The role involves delivering the nursing component of the Healthy Wales Child Programme to a 0-5 age population within a defined area.

Based within the community setting, the practitioner is responsible for the assessment, planning, implementation and evaluation of care offered to families and external organisations, with particular attention to safeguarding children.

Job Responsibilities

  • To manage a caseload within a defined population and lead the delivery & organisation of the public health program.
  • To deliver the nursing component of the Healthy Wales Child Programme to a 0-5 age population within a defined area.
  • To assess, plan, implement and evaluate care offered to families and external organisations, with particular attention to safeguarding children.

Requirements

  • A degree or equivalent in Health Visiting with current NMC registration.
  • Student SCPHN who are due to qualify this year may apply for this post.

Salary: £45,720 - £59,405 per annum
