Job Title: Reablement Support Worker

We are seeking individuals with a genuine passion for helping others to join our team as Reablement Support Workers. As a Reablement Support Worker, you will play a pivotal role in helping customers achieve their goals, working closely with dedicated occupational therapists to co-develop personalised reablement plans.

About the Role

As a Reablement Support Worker, you will provide vital assistance to individuals in their homes post-hospital discharge. You will work on a rota basis between 7am to 10pm 365 days a year, showcasing your flexibility, adaptability, and excellent communication and interpersonal skills.
